Wrong Site Surgery
Wrong site surgery occurs when the wrong body part or location are operated on. Wrong site procedures are relatively uncommon adverse events in health care but are often devastating when they occur. The Veterans Health Administration (VHA) Directive 2004-028 provides specific information on what steps must be taken to ensure that the indicated surgical or invasive procedure is performed on the correct patient, at the correct site, and if applicable with the correct implant.
There are five steps used in the VHA to ensure correct surgery:
- The patient consent form is administered and executed properly,
- The operative site is marked,
- The patient is actively identified using required techniques,
- A “time-out” briefing is conducted in the operating room prior to starting the procedure, and
- Two members of the OR team review pertinent radiological images prior to commencing the surgical procedure.
Specifically, this Directive provides guidance on invasive procedures performed in settings such as patient rooms and intensive care units and will help to ensure such tragedies do not occur in the future.
